carers allowance online application and address to send documents?

Hi,
Long shot but I accidently clicked off the browser after completing my application for carers allowance... on this page it stated the items I needed to send off for them to see, this incl pay slips and pension statements (I think!) - think it also said to write my name and national insurance number on these documents.
I have looked online and can't find the address anywhere, the address on the paper form is different to the one I was given at the end of the application as I am sure the one I was given was a free post address. I don't want to send it to the paper application place in case they cant then pass it on to the online team,
Don't suppose anyone has the address?

When I applied to be my dad's carer after my mum died I just took everything into the local Job Centre and they sent it all off for me. I'd also completed the actual application online. They copied everything I'd included and stamped each copy as proof. I did the same when I started working in June and had to supply payslips to them - don't quite know why as I had cancelled my claim but they wanted proof of earnings - possibly to make sure that my start date & first pay date were the same as I'd told them!
