Setting up direct debit from joint account

Just opened a joint account and need to setup some direct debits. I know the account number and sort code, but what should I enter for the "account holder" box on direct debit forms? Do I put in both our names?
The BT website says I should enter "the name is it appears on my card, statement or cheque book." I don't have a cheque book or statement yet and my card has only my name on it.
When I spoke to Santander, their customer support person said I should just enter "123 account" in the box but that doesn't sound right. What does everyone else do?

Enter the names as per correspondence from Santander, or as they appear on your bank statement, ie
Mr J & Mrs E Smith, or
Mr J Smith and Ms E Brown (of you're not married)

Either name - i.e. the name your BT account is registered on.
In fact it's only the sort code and the account number that matter.0 -
I enter it as it is on my card. It doesn't matter that its only your name.0

We always just put the name of the person setting up the direct debit. Never had a problem0
