    That happened to me in Firefox. I first accepted the terms and conditions (ticked the box below) and the form let me type in afterwards. Weird!

    Thanks - that worked, well at least it let me type in the surname box, still wouldn't let me type in the others - thankfully autofill did the rest for me. :D

    ETA: I still can't enter as there is still no submit button and hitting return didn't work!
