EPA and lack of health & welfare LPA

I've got a big problem coming at me and I need some help, I have epa (originally signed in 2001) over my mothers affairs (she's in a carehome with dementia) and I have been asked by her doctor to consider signing a 'dnr' form, having looked at the epa I've discovered I cannot sign it as the epa is for general affairs only, does anyone know how I can get myself into a position where I can sign and have attorney over her health and welfare ?

I don't know that you need this?
My mother was in hospital recently: my siblings and I were adamant she should be DNR and there was no question of that not being accepted.
We were able to recount conversations where she'd said "I wouldn't want to end up like my sister" (who'd had a bad stroke and is seriously limited in what she can do), which may have helped.
But one doctor did say "yes, she's DNR, it wouldn't work anyway."
Talk to the doctor, ask why they want you to make this decision, and take it from there.Signature removed for peace of mind0
