Just had a look at the British Airways app on my phone, in the "full account details" section it says my card expiry is 31st December 2014.
Is anyone able to tell me whether or not the means I need to act in order to make sure I don't lose the points? Or will I simply be issued with a new card?

You need to add fights or add miles to keep it active. If you are not planning a trip, you can use your tesco points to top up the airmiles, this keeps your card in an active state.Loving the sunny days!0

My clubcard automatically adds points every quarter. Just worried there's a gremlin in the system which may cause my points to be lost.0

It won't affect your Avios unless you've not transacted for three years,
And they'll issue a new card if the account is active (has been used). Not that I've ever had to present my card.
Tier points have their own anniversary date at which point they reset to zero.0 -

Tier points have their own anniversary date at which point they reset to zero.
And the tier points usually reset 1-2 months before the card expiry date (unless you move up a tier, in which case they reset at that point). Membership year runs for 12 months from joining date or date you last went up a tier, card validity usually to the end of the month following membership anniversary.0 -
