Indian Visa Confusion

Afternoon
Google has not enlightened me with conflicting answers so I'm hoping the MSE hivemind can assist.
I'm filling in the visa form and there is a section for previous names. Now - one guide online says you only need fill this in if your name is currently different from birth, another says it's all names previously used. I'm married (but using my maiden name) but I'm also guilty of two previous marriages (oh the follies of youth) and for those I did change my name.
Should I just note those surnames on this form or not?

Normally it is all previous names that you have been known by, so that they can check you didnt do anything naughty when you had a different name, but I cannot say explicitly for the indian visa form0

I put my previous married name down and have never had a problem.
Not sure which guide you're using but take a look at the Goa board on Tripadvisor.
