It should be an ESA3 form. Have your savings decreased from when you last posted?
  • Yes, i now have just under £8,000 so i'm thinking it's now worth doing as the income related top up will also mean that the non-dependant deduction on our rent/council tax will end which will save me around £18 per week. When i claim this, it's only the income related affected by savings isn't it and not my contributions based?

Yes. Your savings don't matter for contributions based ESA. For income based, they will.

  • So when i fill this ESA 3 form in, will they leave me on contributions based and just add this extra income related top up or will filling the form in make them think that i want completely switching to income related?

You can't switch to just income based unless your contributions have run out. If entitled to income based as well, you'd be paid a top up of income based.
