Bank of America Account Trouble

I'm not sure if anyone will be able to advise on this and I think i'm just worrying unnecessarily but if anyone has an answer i'd appreciate it.
I lived in the states for a short time and had a student account (5 years, no fees). Around last August, I withdrew the final $8 from the account, so that the balance was on $0.
Here is what worries me, a few months after that it was due to turn into a regular BOA account. So they would have change it to an account with $12 monthly maintenance fees with a $35 overdraft fee. My worry is that the $0 account got turned into a regular account. It was then charged $12 on a $0 fee, taking it to -$12, which would incur the $35 fee (which is put on every 5 days). Anyway, it's quite feasible that this would quickly run up thousands of dollars.
I've tried logging into the online banking and it says there are no active accounts but my worry is that the new account they opened, wasn't signed up to online banking. In which case there is some rogue account with my name on racking up fees. They haven't tried to contact me to talk about that but I know for a fact that the phone number and addres they have for me would be wrong. However, they haven't tried to e-mail me and they have the correct address.
I've tried calling them a few times (at quite an expense) and have got a range of answers but because i can't properly answer my security questions (due to not knowing the registered address) they can't give me certainty.
Anyone got any idea what banks generally do in this situation?

In the UK it would have been advisable to formally close the account ensuring you received a closing statement. There will be relatively few on here with knowledge of US banking. You might be better on a US site. Have you tried writing to Bank of America.0
