I think this has to do with upbringing. In the same way people are taught to say please and thank you, to eat with their mouths closed, to cover their mouths / nose when they cough / sneeze etc., handing someone their cutlery by the handle without touching the parts that go into their mouths is something that people should be taught by their parents when growing up. Some people just aren't taught to do this so don't know any better.Smiles are as perfect a gift as hugs...
