Capital one ppi claim advice please

Hi
Hope someone can help me, I sent a letter to capital one about mis sold ppi to ask for a refund. They have replied asking me to complete a questionnaire, eg would I get sick pay from work? If yes provide proof, how would I pay without ppi ect ect.
Should I complete this form? Am I legally required?
My complaint is amout mis selling not about how I could make payments without ppi.
Any advice would be greatly appreciate.
