Help with divorce papers

I have applied for a divorce without a solicitor, everything so far has gone fine. But today courts have returned my application for a decree nisi stating the surname in part 2 of the petition is incorrect. I have put my married name in should I have put my maiden name in? It says on the form to put your name exactly as it appears on the marriage certificate. Unfortunately the courts have my marriage certificate so can't check!!
Not sure if posting on the right place, any help appreciated thanks.

    A marriage certificate has the names of both parties before the marriage,i.e. the name in which you married, which is usually your maiden name (but might not be if you have married more than once).

    I've just dug out my paperwork, (my ex applied but I have copies of the paperwork) and on it is my married name, then in brackets "nee single name". Maybe they need the "nee"? Or just phone the court and ask what is on it.

    Had a little look at Internet, seems likely it would be my maiden name. Will phone court on Monday and check whats on the marriage cert. The annoying thing is that they returned some of the paper work but not the section i have filled in wrong or the marriage certificate, both of which i need to correct the mistake! Thanks for advice
