McDonalds parking charge

Hi Everyone and many thanks for all the information regarding parking charges which I have spent the last couple of days reading. I have recently received a PCN from UKPC for overstaying my welcome at our local branch of McDonalds. I appealed the charge but as expected this was rejected and I was supplied with a POPLA reference number. I now wish to lodge an appeal with POPLA but I am very confused as to which template to use. I have been back to the restaurant and spoken to the manager to try to get the charge cancelled, and I am still awaiting an outcome.
I would be grateful if anyone could direct me to the correct template to cut and paste.
