Asking for POPLA code letter template

I cannot find, might not be looking in the the right place, where I can find a letter template to ask for my POPLA code? Don't want to get the wording wrong. Is there a link?

    You don't have to ask for a POPLA code. You appeal as per the newbies thread and then you get sent a POPLA code if they reject.

    PLEASE PLEASE stop starting new threads, I covered this yesterday. We can't handle a new thread every time you need to ask something. You just hit reply on your own thread each time and your thread is bumped back to the top.

    Please send a pm to Crabman to merge all your threads.
