Firstly, it's great you are taking steps to help your situation. Secondly, I wish your partner all the best of health.
Have you included all debts? You could write a list of all debts (creditor name, account number and amount owed) to see exactly what you need to sort. Then you need to do a chart of your in comings and outgoings. When it comes to outgoings include everything. Even things like birthdays. Then you can look at what money you have spare and what you can pay to creditors.
There are free debt advice companies who can help. Payplan and Stepchange are two. They are great with advice on how to deal with debt and what avenue you can take. Have you thought of going down the debt management plan route?
Whatever happens, don't stop making payments but do contact the creditors to explain you are experiencing financial difficulty.
Honesty, once I made contact with payplan, I felt assured and in control of things for the first time in years. I really wish you both all the best. Good luck!
