Either go to a company registration site or engage an accountant and most will do the set up charges at cost or for no additional to their normal annual fees.
Now if there is any benefit to Ltds to a nurse is another question but not the one you asked0 -
There may be IR35 issues with what you are proposing. Whether you can be self employed or not is determined by the work you are doing and is not a free choice. Are you going to be working for several employers?

Go about registering what? A limited company?
Companies House is responsible for registering all limited companies. You can either do that directly with them, or as already advised, use an agent (which is often easier, quicker & cheaper) or even ask your accountant to do it for you (who'll probably use an agent anyway)0 -
