Halifax TMPP letter re: premium refunds

DH received a letter yesterday from St Andrews insurance, quoting a TMPP account number from his old halifax mortgage with his ex which ran from 1996-2005. They have stated he may be due for a refund of premiums and have asked for his certified ID photocopies, which he has done in branch today.
I gather that they are referring to a PPI segment of the TMPP. DH does not remember anything, he has always thought he never had PPI. The TMPP premium was taken with the mortgage payment, (not a Separate payment) thought he was paying for life ins/critical illness cover.
We are just wondering what the premiums tended to be with TMPP for the PPI segment? Was it a percentage or a set fee?
