Change of Address, or better how to cancel LLoyds Avios Cards

Hi all,
I am failing at changing my address with my Lloyds Duos Avios card, there is no form online, some addresses on the web (where to eventually send a form) are old (quoting TSB).
I've called their customer service and they promised to send me a form, which never arrived.
I am quite fed up with this, this is the 3rd time I have go trough this weird procedure to just change one address. Most cards (and I have few) offer online address change, it's pretty simple.
So: how to cancel the cards with them? I Had enough!

Sorry to hear of the trouble you've had. To change your address by phone, you need to be registered for Telephone Banking. If you're not registered, you can do so by visiting your local branch or by sending your request in writing to:
Lloyds Bank
Box 1
BX1 1LT
If you still wish to close the account, you may wish to include this request in your letter.

I'd hold on a second, if you are using the cards(opposed to using them for one off emergencies etc), don't cancel them as you probably get them free of charge, and Lloyds now charge £24 per annum for the rewards.0

is there an official form somewhere to send over to just change my address?
Happy to craft it myself, but, where should I send it?

There's not a form for credit cards, so just send a letter to us with your details, including your credit card number (the one shown on your American Express card). Please also include your signature.
The letter should be sent to:
Lloyds Bank
Box 1
BX1 1LT
