hi survey is still there for me scroll down to bottom of page, but quite a few of the questions are redirecting me to h&m website? not sure if its just my computer. still let me submit with some questions left blank though.

As mentioned above the survey questions started under the band at the bottom.

There are three or four questions on each survey page, so click next for the next set. The final couple of questions are for your contact details- name and email.

I had the same problem. Almost the only questions I could answer were the last ones on each page. Still, it accepted my details, so fingers crossed.
