  • Thanks for the info, much appreciated. However my issue is despite it being done from medical records, information is missing.

    Also on one part of the ESA113, it asks about sitting, standing, walking, reaching and other issues, how is a GP that i've never seen before supposed to answer correctly because if he states I'm ok to do all those things then he isn't being accurate and if it affected my assessment then I'd lodge an appeal and that form would be one of my grounds for doing so.

    I had to make an official complaint to the practice manager because of this, although i've emailed consultants letters to cover myself just incase ATOS try saying i haven't got this or that because it's not listed on the form.

    I have multiple health problems, I have all what you have and more, and because of that, I spoke with the practice manager as I was being passed to locum doctors, which was just a shambles as I kept having to repeat my medical history. After I spoke with the practice manager, I had my own named GP, and have been seeing him regularly. He knows me, he knows what I can and can't do, he know's my history and I attend the surgery often.

    Do you not have a specific GP? Did you get a GP's report? I paid for mine £50. Do you attend the surgery often or infrequently?

    If you don't go to your GP, the DWP will just ask anyone in the practice to complete it, and have been told it's mostly a tick box exercise, hence my asking for a specific report. Have you contacted your surgery to see the report? You are allowed to see it, as it's part of your medical records. You are entitled to get a copy of that DWP assessment from your GP for a small fee. Have you tried that?

  • Hi, I've seen the first part of the form where it states about my conditions and quite a few things are missing.

    Also, the information should be on the medical records because all the consultants letters are sent to the GP, if there are things missing then that's the fault of the surgery. It needs to be corrected though. I plan to change surgery within the next few weeks because if seems to be one mistake after another over the last few months with medications, prescriptions etc so I think it's for the best.

    Hopefully sending consultants letters overrides some of the ESA113 as they can't deny things that i've been diagnosed with by consultants.

    Just to give you a heads up. If you're changing surgeries, it can take up to 4 weeks for all your notes and records to be passed over, so you may want to take that account if you're disputing that report. You can actually have copies of what's in your medical file, it usually is about £60, less if you just want to see what is in there. But keep that in mind that your new GP will not be able to do anything until (s)he received you notes, which may take a month to get.
