Explaining Power of Attorney to elderly parent

Can anyone point me to a VERY basic explanation of Power of Attorney - both Finance & Health. - to give to my elderly father aged 90 to read.

My brother & I already handle his affairs under his direction [there's nothing wrong with his mind] , but he's crippled with arthritis and finding it very difficult to sign the occaisonal cheque and other paperwork. Also considering his age, I would be happier with something in place regarding what do do if, e.g. he has a stroke.

I would like to formalise the current arrangement and have things in hand should his condition deterioate. He lives alone in rented accomodation opposite me, so no property is involved, just his savings.
