Help needed for the feeble minded

Have issued a claim for bank charges through mcol and cut and pasted the wording for the claim from Martins letters, however being totally stupid I forgot to include details such as account nuber or reference number. I also issued claim in my name but it is joint account. Have I just totally wasted my money or is there any hope left for me.
I know it is probably to late to change claim but does anyone think there might be something else I could do?
Any advice would be most welcome
