Case Management Info Sheet - Merc Courts

Hi all, got home tonight to find a letter from Leeds Courthouse re my bank charges, in it is a Case Management Sheet asking for all sorts of info, is this basically my court bundle as it ask things like:

Substance of case - no more than 20 words
Statements of case - Do I intend to amend my statement of case see CPR18
Disclosure - By what date can I give a standard disclosure, mentiones CPR31.12
Admissions - Can I make any additional admissions
Preliminary Issues - Any issues suitable for preliminary trial
Witnesses of Fact - Do I intend to rely on witnesses?
Expert evdience - any required, if yes what
Trial -
Additional Dispute Resolution - has it been considered
Other Applications - What do I estimate costs to be to date & at the end

Wasnt really expecting all this waffle but hey it's my first tie, any help would be appreciated?
