Contract termination due to long term ill health

Hi,
I've been employed for 11 years 3 months. Unfortunately I have been on long term sick for almost 6 years. I've attended numerous medicals arranged through work which have all supported my consultant and GP diagnoses.
I was eventually called in for a meeting today with HR for my first meeting in over 4 years of face to face meetings - have only had very sporadic email contact during this time - all initiated by me!
I have been told that the next stage is a formal meeting, where I assume they will be terminating my contract as there is no sign of my health improving.
My question is, what can I expect from this meeting and what entitlement will I have, if any, to a termination package - I am aware that my company have only offered redundancy packages on the bare minimum amount required, so do not hold out much hope!
Any advice etc, gratefully received as I am finding it very hard to gain any answers or advice as to what my legal rights are.

Any idea how to move it / start under the work section please?
Tried Citizens advice and ACAS and they couldn't really help. :-(
