HSBS PPI Claim - 2 claim forms or 1?

hi, I called HSBC to find details of my previous loans and PPI / loan protection cover. I was told I had 3 loans but they couldnt give me dates for them. They also said there were 2 references for Loan Protection.

Do I complete one PPI claim form to HSBC giving what little details I have for both numbers or do I need to do 2 forms?

Also without the dates and without them being able to tell me which reference related to which loan its difficult to give specific info for each one. Should I just tell them all I know in one form? What should I do?

They relate to loans over 6yrs old. Thanks!

    HSBC require a complaint form for each loan. However, if the Bank can't provide the dates of these loans, it doesn't sound as if they have full records which may mean you encounter problems with any complaint unless you can provide evidence from your own archive..
