General form of judgement or order

i have received this form . i understand most of it like i have to send a list of charges to the court. Q is it just one list?
Defendant to file and serve within 14 days of service of this order a statement setting out all facts and matter relied upon as to the banks actual loss as set out on para 11 of the defence, What does this mean? do they have to say what it has cost them?
Then i have to refer back to district judge after 21 days, What does this mean?

Send a list of charges with charge type, date, amount and a total to the court. The court is also asking for the defendant to disclose costs. This is excellent news for you, as it's very unlikely they will do this.
