CPP Policy Holder Deceased

My mum and dad always had joint bank accounts. My dad passed away two years ago but I knew they had paid to CPP. I got my mum to telephone them today (using the number from this site) and they checked the postcode and found my dad was registered as a policy holder. My mum asked them to send another letter.

How should my mum fill this in? She could tell them my dad is deceased but she is his surviving spouse. She could just forge my dad's signature on the form, but this may mean any cheque is payable to him.

Any suggestions please.

    The cheque can be paid to the estate and distributed accordingly. Don't forge signatures.

    You may need a copy of the death certificate to put with the claim.

    I have a copy of the death certificate and was considering sending one. The bank account any CPP related to was also a joint ac!!!!, so not sure why only my dad could be the policy holder. Just want to make sure my mum gets what she is probably entitled to.

    Make sure to return the forms as soon as they are received. The CPP refund scheme ceases at the end of this month.
