Urgent help needed with UK passport application

My gf bought me and her son and her mum a holiday to Ibiza on September 12th. She still doesn't have her passport but she's filled in all the paperwork and got her and her sons witnessed but we need help with a few things.
Firstly her original passport got stolen many years ago..so the post office said fill in a lost passport form and attach it. But on the lost form its asking for details such as what police station, crime ref number, original passport number. As it was over 10 years ago when she lost it she doesn't remember any of these details, so can she leave these blank or will her application be rejected?
Secondly, we live in Sheffield and due to the timeframe we are thinking that fast track 1 day passport express option is the best option, but her son is getting his first passport she is doing a renewal as such..so..apparently this isn't available on a one day service for children..so it means we'd have to do both in 1 week, is it a interview/appointment and then you get your passport in a week? From what I read online this is what I believed. Do you know wheres the nearest centre for it? Just I looked online and it said there is one in sheffield but our relatives seem to think we are dreaming..Also as we fly in 40 days do you think we will get the passports in time?
Also we noticed that the book says you can't mistake more than 3 mistakes per 'line' does this mean per box line ?
Moreover on the child's passport section I think it's section 4 it states if your born after 1982 you need to require grandparents details? Is that from both the and dad's grandparents or just the grandparents of the parent filling hot in ? Also we've read mixed stuff that..some people say you can just provide name, date of birth and marriage etc etc and passport number..whilst some others are saying you need the gparents identification and have to bring it?
We are going to ring tomorrow and are hoping we can get an appointment in time so we get the passports in time?

    There is no interview for a childs first passport, only adult first passports have interviews. For some accelerated schemes there are appointments but having used the same day service, all they do at the appointment is check the form and photos and tell you what time to come back to collect it.

    Not aware of needing any grandparents ID, just entering their details. Of cause these things do change over time. If you do need it then the guidance notes should tell you that you do.

    https://www.gov.uk/ips-regional-passport-office tells you where the nearest one is. I know when my wife did her first british passport the length of waits for appointments varied massively, london was a 2 week wait, another was a week, Southampton could do the next day.

    I got my daughter's first passport on Friday after 5 weeks that I sent via post. For my first child, I used the fast track option of 7 days and got the passport within 4 days.

    To answer your other questions:

    1. It should have been reported when It was stolen but since it was not done, I am not aware of the repercussions.

    2. Use the link provided by InsideInsurance to find your nearest passport office.

    3. I think it is 3 mistakes in the entire form (I could be wrong but I never did any :))

    4. Both of my children born after 82 (2011 and 2014) and I didn't provide any grandparents details. I just provided their Full Birth Certificates and my passport.

    Not much help to you now but if anyone else is unsure of the whole providing grandparents information and what you should do then i actually spoke to the passport office and found out the following:

    If both parents of the applicant were born after 1982 then you need to provide Grandparents details, i was told names, dates of birth, place of birth and dates of marriage would be sufficient.

    However if you can provide passport numbers for both of the applicants parents then Grandparent inforamtion is not actually needed.

    It's all a bit confusing but if you read table D in the info booklet it does actually have a small section on the Grandparents details.
