If you are worried, remember that although to you it looks like a catalogue of problems with lots of points where you "should" have done something different earlier, bit to the OR it is going to be pretty normal story of someone going bankrupt in 2014.0 -
Should I have to put in every time I consolidated? Think I did it 3 times over the years but not sure when and no record of those bank details due to my account now suspended so can't access them online. Want as much info for OR as possible but not that easy it would seem0

Does not have to be a super accurate timeline of events. This is basically so the judge can see how you got to where you are and your situation, and to give the examiner a starting point and good heads up for any interview. As such you should certainly put that you consolidated several times, as it's an important part of how things came to pass, but dates and exact details are not crucial on the BR forms at least.
