Tax related question

Hello,
I was hoping some of you lovely people could give me some advice/steer me in the right direction...
I was working PT at a school as a teacher, and PT at a supply agency. I was lucky enough to be offered a full time contract by my school, so I handed in my noticed at the supply agency.
I received an email confirming they received and accepted my resignation. I was assured I would be removed from all their books. A year a later I was still receiving the odd text/missed call from them asking if I was available to take on a booking! :mad:
I contacted them again and was assured I wasn't on their books! Pah! The calls/texts stopped after, which was positive... but then I received a letter from HMRC with my new tax codeS
I then twigged I hadn't had a P45 from them (my stupid fault!):embarasse so chased them up for it, and they were adamant they'd sent one out to me... but agreed to send me a digital version.
My question (we got there in the end! Thank you for sticking with it!) is, how do I notify HMRC that I need to change my tax code? (My current code for 2014/2015 is for someone with two jobs)
I have been on the HMRC website and filled out an 'incorrect code' form, but it doesn't seem substantial enough, I think I was expecting to have to attach my P45 to the form...
Any help would be greatly appreciated.

Just a quickie but a copy of the original P45, as well as being sent to you, one would have been sent to HMRC.make the most of it, we are only here for the weekend.

This is what I thought, however, the date I ended my contract with my supply agency was dated 2011, yet I still received 2 different PAYE tax codes for 2014-2015?! Mind boggles!0 -

Did you have two Tax Codes for 2012-2013 ?0 -
there isnt a specific tax code for someone with 2 hobs.
in those circumstances you will have the usual 1000l code for your main job and a BR ( dasic rate) for the additional job.
so unless you earn money at the second job, you only pay the usual amount of tax

Decided to just ring them up and get it sorted. Apparently they only received my P45 a couple of months ago! (Interesting!) and I'd over paid by £20! Tax code changed and all sorted! Thanks for your help.0
