Cancelling O2 contract dispute

I've not used an old mobile phone I had for a year, and have been paying (daft, I know), however via their live chat, I requested they cancel and they didn't. Turns out - and O2 acknowledge this - the chat was cut off (by them, not me!).

Am I liable for a refund of any more money I've paid to them after this date?

The reason is - I've requested in writing my cancellation - and they "didn't complete the process".

Is that their fault or mine?

Can I do anything to legally reclaim any money I've paid while not even using the phone?

My original contract term ended about 18 months ago!
