    Your own bank will probably do them for nothing at the enquiries desk, mine (Lloyds) always does. Just always make sure the original never leaves your sight, as it's completely irreplaceable!

    Not probably, definitely! Maybe it wouldn't be obvious from the paperwork but, if it came to the crunch, the witness would have to give the game away or perjure himself.
