    What about a balloon in a box if you want a physical gift. I remember buying one for my grandma and she loved it.

    I second the balloon idea ... my mum was in a hospice for her birthday, and even though she wasn't 'with it' most of the time by then, a couple of bright cheerful balloons tied to the end of her bed were appreciated (and not just by her).

    In the year that my Grandpa was in a home, he always appreciated flowers that I'd grown in the garden (and then giving me the benefit of some of his gardening experience) but probably most of all, our time

    When my Grandad was dying, we bought him an mp3 player and stuck all of his old favourites on to it so he could listen to music in bed.
