Details for PPI

I have a letter from the bank asking for details regarding PPI on a loan.
Over the years my ex and I have had a few loans but generally refused all insurance as he was self employed and in any case it would push the price up too much, so surprised to find out one slipped through.
Anyway the bank who have the policy number on the top of the letter for this loan are asking questions I have no Idea how to answer ie personal circumstances at the time of the loan, all I do know is we would not have taken PPI if we hadn't been under the impression it was obligatory.
I have no details of any past loans in order to answer any of the questions on the form but clearly they do, can I move forward with this or just forget about it?

    Just answer truthfully. If you don't know or can't remember, just say so.

    So just saying that I know we wouldn't choose PPI and therefore must have been mis sold is enough? Doesn't matter that I can't fill any of their form in? Can I phone them and ask the details of the loan as that might help to fill out questions ?

    It's a very weak hearsay complaint for which you can provide no evidence, but whether it is "enough" to produce a refund of your insurance no one but the bank can say. The Bank may find other errors with the original sale as part of their investigation or they may not.
