Littlewoods PPI Reclaim

Hi All,
Sorry to drag this up again but have been round the site and can't find an answer.
I have had PPI on a now closed Littlewoods account. I have some statements with amounts paid and my account number and the last address the account was at. I do not have the dates the PPI started or ended.
My question is do I need a SAR with full PPI amounts paid, before I lodge the claim, or can I just use the limited info?
Also, I am no longer at the address of the last statement - how do I change that? Is it acceptable to send the change of address in with the claim?
