what form maternity leave pay?

Could someone please tell me what form do I need to get from Doctor/Midwife to apply for maternity leave pay? And, do they have dead line for that?

MatB1, should be handed in to your employer by the time you are 25 weeks pregnant x0

It's normally issued to you by your midwife when you are about 20 weeks pregnant x:heartpuls Daughter born January 2012 :heartpuls Son born February 2014 :heartpuls

MATB1s legally can't be issued until you are 20 weeks pregnant
You don't have to give it to the employer by 25 weeks (although you are meant to have informed them when the baby is due and when you want to start maternity leave by then and many employers will ask for the MATB1 at the same time) it's about 28 days before the start of mat leave0 -
