I had turd laid some 2 months ago but it has never really taken that well. The guys who laid it didn't prepare the ground beforehand, in that there are still stones, debris, nails left over from the house build if I peel some of the turf back.

Is this a result of poor maintenance (I haven't done any as no after-care instructions were provided), bad ground preparation or both?

    Yes

    Newly laid turf needs water - lots of it.

    Water , and then water again .
    I mean turn the sprinkler on for 15 mins ( min) EVERY night for a couple of weeks.
    Don't stand on the lawn, that means starting the sprinkler furthers from the tap and after the required watering , pulling the sprinkler to the next section
    The lawn should knit to the soil in about three weeks
    Reading your opening post , I hope you didn't pay a lot for your lawn.
    My cost ( 2011 ) was £600 for about 110 sq yds.

    Seriously, that's close to 'start again' bad, with proper preparation of the soil and a decent turf not from a farmer's field.
