Thanks,I thought it was just me,i emailed and sent an enquiry and an email saying i hadnt got it,then you have to go to the questionnairre and just keep pressing next page without answering and click to say you have copleted this bit.when the whole trial finishes and we answer the final questions it will have a box at the end saying why couldnt you answer the first questions,this is twice in 2 wks for me now.

Yes I was down to trial the eye treatmet, I assumed it was lost in the post or the trial date had been postponed.
