PPI claim form from MBNA

Have just recieved a claim form from MBNA ....stuck on section 1 though, it gives 3 options at the beginning of the form..
1 age
2 Employment circumstances
3 Residency
it includes boxes to the right of 1 (age,insert DOB), 2, employment circumstances (insert details of type of prsi, or p45, or social welfare payments)...
its very confusing to understand what makes me eligible for a refund...also for the 3rd (residency) it gives no follow up box for details...

anyone here give me some guidance or enlighten me as to direction to take, they say at the start of the form that the review process is easy to understand and free, .....bit patronising to say the least :mad:

    You do not become "eligible" for a refund.

    If you can show that the insurance was mis-sold to you in some way then the Bank will refund any PPI and associated interest.

    That's a very weak "hearsay" complaint. What evidence do you have that you were told it was mandatory when it wasn't?
