Bridesmaid dress not acceptable standard - advice please

I arranged for a childs bridesmaid dress to be made by a lady based in London selling from her own website, Facebook, Etsy and Notonthehighstreet sites. Her communication was friendly, helpful and her reviews were all good.
Unfortunately when the dress arrived it was terribly made, the gathering on the skirt was uneven, the seams on the bodice/lining did not match up, the length did not seem right and it just looked awful. I complained and was told that the person who had made it had been diagnosed with a brain tumour and that she hadn't checked the dress before it was sent out.
She did not apologise and her communication changed - I only got information when I pushed for it and she would not give me a timescale for putting it right. I had to send the dress back and eventually she told me they were making a new bodice for the dress and using the original skirt.
Eventually I have received it back and the bodice and gathering is much better..... but... in order to make the dress the correct length she has undone the original deep hem, let the dress down and done the smallest hem possible on the dress. The fabric is silk and therefore you can not only see the press mark around the dress from the original length, but the needle holes from the original stitching.
This is not acceptable to me.
I wanted to find out what I would be able to do about this? Can I send the dress back and ask for a refund? Given that she has had one chance to put this right?
Any advice about how to proceed would be gratefully appreciated. I have had the dress over a week now as I have been too upset to do anything, I hope that this doesn't mean I have left it too late.

Contact her in writing and explain factually what is still wrong. I would give her a few options if possible.
A) full refund within 14 days of dress being returnedrepair as per agreed design
C) partial refund of X% to compensate for faults.Should've = Should HAVE (not 'of')

Hannah did you get a refund? as i'm having similar problems my dresses have come from America and the lady is refusing a refund. i'm thinking of opening a case against her0

susie, start your own thread. If you bought from America, then you are bound by their laws, not ours.One important thing to remember is that when you get to the end of this sentence, you'll realise it's just my sig.0
