I will just if I may remind you of the first thing I was taught in Computer Studies at the end of the 60s - GIGO. Never a truer word. You mentioned systems and their operation. The weakness is neither. It is the humans and companies using them negligently that cause the problems. I'd even go as far as to say we are talking criminal negligence by those who are officially let loose on them and on our personal data.0 -

This isn't particularly about IT, though. These fundamental breakdowns of system logic have the potential to exist whether systems are IT-, people- or paper-powered.
Where the systems are owned by multiple corporations who do not communicate with each other to any great extent, it is surprising that the issues are not more common.0
