Emailing a form for people to complete

What's the best way to do this? I need to email a blank form to get people to complete, nothing involved like a job app form, just a simple table for them to put a bit of info into and send back.
I thought to add a word attachment with the blank form but then people have to save that, complete it and attach it back to a reply and with so many using tablets and phones for emails, that could be fiddly and I know from past experience you often get blank forms back.
The other alternative is to send the table as part of the body of the email for them to complete and return. I think this might work (have tried a dummy run) but not sure if tablets and phones can handle it.
Can anyone advise on the best method please? Or something else I haven't thought of?

I suppose the "best" would be an online database type of thing
like one of those customer purchase survey feedback things

Do you have Microsoft account (Hotmail or whatever)?
If so you can use this trick to produce a survey, which will save the answers in an online Excel spreadsheet. It's free, and you don't need a Hotmail address to get a Microsoft Account if you don't have one already.
To get there, if you have a Microsoft Account, goto Onedrive.live.com and use the Create option to create an Excel survey.

You could also have a look at https://www.surveymonkey.com which I've used a few times in the past for this sort of thing.0
