Simply Switch claim form

Hi, first time poster so please be gentle with me on this. Also I have tried looking through previous threads and can't find what I'm looking for so apologies if this has been covered before and I've missed it.
I switched my gas and electric last night using the Simply Switch link on MSE and received an email from Simply Switch saying my switch was successful, qualified for the cashback and that all I had to do was click a link for the claim form. Unfortunately the link comes back as an 404 - File or directory not found.
The resource you are looking for might have been removed, had its name changed, or is temporarily unavailable.
Does anyone out there have a copy they have they could send me? I haven't tried their customer services yet admittedly primarily because of previous threads from people chasing their cashback last year which describe their Customer Services department as all be asleep.

Or if that's a bit too much too ask does anybody have a working link to the claim form? That would equally be as helpful.
Have bitten the bullet and emailed their customer service department also requesting the form or a working link to it. Wish me luck with that one.0 -

Seems even their most current emails have the incorrect URL. As posted above, the correct URL is: https://energy.simplyswitch.com/claim/cashbackform
Unfortunately both their T&Cs as well as their e-mail to customers asking them to fill in the form goes to the incorrect URL of: https://www.simplyswitch.com/claim/cashbackform
(i.e. the 'www' should be replaced with 'energy')0
