Form filling app for ipad?

I have some forms to fill in and send back by email. I've been looking at apps that do this but they seem very expensive for something I'll not use much. I need to be able to tick boxes and sign my name. Can anyone recommend a free or cheap app that will do this? Most important is that it's easy to use, preferably idiot proof! I've tried PDF reader but cannot work out how to use it and ended up drawing lines all over the place. :doh::doh:
