Lloyd's PPI questionnaire received

Hi all, I would appreciate any help as today I have received a form from Lloyd's bank regarding a loan I had with them. They have asked me to fill it out as I have potentially been mis-sold PPI for this loan. I'm confused however as to what to do next, do I just need to fill out this questionnaire and send it back, what do I put where it asks me why I think I've been mis-sold the PPI, what information I was given before I took out the insurance, what questions I asked etc? To be honest I can't remember as it's nearly 6years ago much about the conversation and have little if any paperwork still for the loan, Do I need to add a covering letter? Would appreciate any feedback especially from anyone who has dealt with his form from Lloyds. Many thanks in advance

I'm going to, I wasn't even aware I had PPI on this loan to be honest, but figured now I've been told I have I have nothing to loose by filling out the forms(when I work out how to!) Was just confused as there is a whole a4 page to add additional information on what info I was given and how it was explained to me at the time, and I'm not sure
