What do you think has been done wrong and by who?

Your answer will dictate who you send the form to.

There is nothing wrong with simply taking insurance.

Check old bank statements to see who your lenders were as you may have paid them by direct debit, see if there's account numbers quoted next to each transaction. Then phone the business and ask if they still have any records about you, if not, you may need to send a SAR, so they can dig a bit deeper.

No need for any of this. All you needti do is write to whoever you think has done something wrong telling then what you think has been done wrong, when and by who.
