iphone 4 itunes problem

when I updated my iPhone 4 ages ago, my iTunes has never been the same. Keeps skipping songs and has symbols after the songs. I have been told to completely remove all the songs and then resync again. But I have no idea how to do this. Can anyone help me in really basic steps please?
