I have been sent a claim form by CPP to make a claim for a mis-sold identity protection policy. The name on the form is my maiden name as I took out the policy before I got married. Do I complete the form and sign in my maiden name or can I change it to my married name. Concerned that if they make a payment to me in my maiden name I won't be able to pay it in to my bank? Appreciate any advice

I sent a copy of my marriage certificate to cpp and they sent me a new claim form in my new name. You have plenty of time to send for a new form and fill it in with your correct name.
