Getting Drs records

Hi
not too sure if this is the right forum, but anyway
If I put in a request for my Drs medical notes, will it come up with ,when i have changed GP within that practice.
Will the notes come up with any Drs who has accessed my records

It may depend on the IT system they use. At the last place I worked the answers would have been maybe (but probably not), and no. It gets further complicated by some things which can be on the record when you view on a computer and are hidden on print outs.0

If your concern is that a doctor has accessed your records when they had no clinical need to and for improper reasons, this is something the Information Commissioner should investigate for you. Why not give them a call and discuss it with them? They are quite helpful with the public and if they find that your complaint is upheld, the fines they are currently dishing out are quite hefty.0

depends on the system used - some show any access in the main record, others will only show it if an audit trail has been run.0

Normally within a practice, the records will show which GP has seen or advised a patient. Normally all of the records will be available for all doctors, and most clinical staff at the practice to look at.
The way in which most group practices operate would assume that any / all of the doctors might need to access the notes.
If there is an issue about a particular GP seeing someone's notes, then on the whole that patient would be best off at another surgery; but I have known of patients asking not to see a particular GP. Best to speak to the practice manager initially.0 -
Newer versions of EMIS do allow you to lock notes down to doctors only rather than all staff, though I've only seen that used where staff have been registered at the practice they work at.0

Why do you specifically need a nurse if it is a non-medical question?
You can't stop staff who have a responsibility to refer, file, or treat patients looking at a patient file if they are registered there.0 -
Contact your Practice Manager and make a request to see your medical records. You'll need to fill in a form, and there may be a small charge, £10 or so, plus a bit more if you want copies of anything. Any entries on your record will be attributed to someone, but your Practice Manager will be able to explain more.0

If there are paper files involved it can be £50 rather than £10 as per http://ico.org.uk/for_the_public/personal_information
To the OP - by the sounds of it you want something broader than the traditional request to see your medical records. It may or may not be captured depending on their IT systems but in your SARs request you need to explicitly state you want their response to include any data around access of your records.0
