I am signed up to a number of restaurant mailing lists and find that around the month of my birthday they usually send me special offers such as free bottle of wine etc.

sizzler pubs offered me free starters if i go there for a meal, plenty of other offers through the year too - well worth signing up if you have a place nearby.

Beefeater send you a free main meal voucher for your birthday if you get a loyalty card.

If you use wifi in a harvester restaurant they send you a voucher for a free pudding when you buy a main, at each restaurant you've used wifi in.
(it's my birthday next week and I got 3 through about a week ago)

Frankie and bennys you can sign up to their birthday club get a different offer depending on child or adult... though I haven't seen that one yet this year...

Chef and brewer pubs just to do something - again not received yet this year but used it for dads birthday last year...
