it was mentioned on tv that there was an official form that has been sent out to everyone concerning PPI reclaims !! they showed it but I cannot remember what it looks like does anyone know what it looks like or where I can see a copy so I fill in the right form

That is not correct. Some firms are sending out client care letters (CCL) that let people know and invite them to complain if they feel they have reason to complaint.
There is a standardised form that can be used and its available on this site here:
